According to military sources, Iran likely had an immediate plan to launch ballistic missiles. But all those who could have given the launch order were eliminated. The missile launchers were placed on constant combat readiness to deliver immediate strikes on Israeli military bases and prevent possible attacks on Iran's nuclear facilities. That's why Israel first eliminated the top military and command leadership in Iran.
According to Israeli sources, even before Tehran could recover, the missile launchers that were not affected by the initial strike were also destroyed. So there was no one left who could issue the order to launch the missiles towards Israel.
The inflicted damage seriously impaired Iran's ability to respond to the attack. According to a report in the "New York Times", Supreme Leader Ali Khamenei ordered the launch of up to 1000 ballistic missiles against Israel in retaliation. But the destruction of the missile bases limited Iran to less than 100 missiles in the first waves of the counterattack.
Iran had been preparing for more than a week for a possible Israeli attack if the nuclear deal negotiations with the US failed. But the regime did not expect this to happen before the planned meeting in Oman on Sunday. The top leaders thought the warnings of an impending attack were propaganda, an attempt by Israel to force them to make concessions.
It seems that due to this overconfidence, the security measures were not effective enough - and the leading commanders, left unprotected, were eliminated by Israel in the initial phase of the operation. In internal correspondences, senior Iranians indignantly asked: "Where is our air defense?" and "How can Israel come and attack whatever it wants, kill our senior commanders - and we can't stop this?"
