The Municipality of Nessebar has launched an ambitious project, funded by the European Union, to provide integrated health and social services in the home environment for people living alone with disabilities and elderly people unable to care for themselves. The project includes the development of an innovative model for remote monitoring of the health and psychosocial status of users through modern technologies such as telecare.

The project "Innovative Health and Social Services in the Municipality of Nessebar" is aimed at improving the provision of integrated health and social services in home environment for persons living alone with disabilities and elderly people unable to self-care. The project, funded by the Human Resources Development Program 2021-2027 and co-financed by the European Union, aims to build and apply an innovative model for remote monitoring of the health and psychosocial condition of users, based on modern information and communication technologies - telecare.

Telecare is an innovative telemedicine service that provides continuous monitoring and immediate response when needed for lonely elderly people in vulnerable situations. It detects early signs of health problems and reduces the need for hospital treatment. "Smart bracelets" that automatically measure vital signs, provide two-way communication, SOS connection and GPS tracking will be provided to 5 users.

The project also envisages the provision of hourly mobile integrated health and social services from specialists such as therapists, nurses and home assistants. Activities include meeting the daily needs of users, as well as assistance in accessing other health and social services. The indicative number of users is 64 persons who must meet the conditions for living alone with disabilities and living alone in retirement age unable to self-care.

The project will be implemented for 24 months, and the amount of the grant is 640,619.27 BGN. The provision of hourly integrated health and social services and the use of telecare devices will be free of monthly user fees for those in need.

The project plays an important role in preventing social exclusion, ensuring that vulnerable population groups will receive timely and professional support according to their individual needs.
